- • IOA Includes: Consultation Intake Evaluation, Initial Session and Regenerative Session Specific to you
- • PH Consult (PHC) $295-$350 (45-60 Minutes)
Both IOA & PHC Include: Specific Plan of Care (POC)
• Recommended before your Initial IOA:

- B) Send any Lab results you have, or your My Charts URL, Username/Password to dp@21ctw.com
- C) Your New Client paperwork available at: https://www.21ctw.com/contact/ can be dropped off in our Black Box by Front office door (text 219-613-1161 which date and approx. time before dropping off).

You can also bring in during Initial IOA.
2) Routine Clients: In Office Appointment (IOA) $250-$325 (45-65 minutes)
PH Consult ($65) to $175 – $250
3) Home Evaluation & Regenerative Session $550-$725 (this is approx. 75-90 minutes, prepaid, and by appointment only) Mileage cost determine separately
4) Tele Medicine/Health: Phone Consults &/or Text/email communication
Routine Clients 30 days (M-F Normal HRS) of Tele Medicine Text/Email $105-$250
